Attention to Our Students Who Want to Apply for the Summer Term Internship for the 2021-2022 Academic Year.

A) INTERNSHIP APPLICATION, INTERNSHIP START and END DATES
Internship Application Dates : 09 – 27 MAY 2022 between the dates.
Submission of Internship Start Documents : 03 JUNE 2022 (Last)
Internship Start Date : 27 JUNE 2022
Internship Start-End Dates : 27 JUNE 2022 -05 August 2022
NOTE 1: Due to Covid-19 illness, it would be healthier for our students who will send the requested documents below via cargo or e-mail to contact their class advisor, internship commission president, or student affairs. (Note; Those who will send to the e-mail address specified on our website or by cargo should send it to the campus address.)
Note 2(IMPORTANT): 1st-year students who have taken all the courses of the first two semesters at least once and fulfilled the attendance requirement can optionally do an internship at the end of the 1st year, considering their justified reasons and that the student's professional development is not adversely affected, with the approval of the relevant program internship commission. 1st-year students who meet the necessary conditions and want to start an internship must apply with the petition attached. Students whose petitions are accepted by the commission will then be able to start the internship application procedures.
B) Completion of Internship Start Documents:
1. Internship Acceptance Form (Appendix-2) will be used for application to the institution where the internship will be done. Before giving it to the institution, the part where the student's name is written will be filled in and the date selection will be made, and a student certificate will be attached to the letter. (The student certificate will be obtained from e-government)
2. After the student information part of the Internship Acceptance Form (Appendix-2) is filled in, the student signature, institution/employer signature, and advisor signature will be completed, and 3 (Three) copies will be prepared. One copy will be delivered to student affairs, one will remain with the student, and the other will be delivered to the institution/employer where the internship will be done.
3. If the institution/employer where the internship will be done accepts, the internship information will be entered into the OBIS internship information section. (The process of entering internship information is explained in detail in the OBIS operations link.)
4. After the OBIS internship information entry is made, student affairs control and approval will be done.
5. The Student Health Insurance Declaration Petition (Appendix-3) will be filled in and signed. (To specify the Student Health Insurance, a barcode document will be created from the Social Security Institution/SPAS Eligibility Inquiry (Health Provision Activation System) via E-Government and delivered to our school among the application documents.)
6- Before starting the internship, students who will go on an internship must add the Occupational Health and Safety Certificate (Class A) if they have obtained it. A copy of the relevant certificate will be delivered to our school before the internship starts. (Those who have taken the online course provided by our university or from occupational health and safety centers).
Note: For the State Contribution to be Paid to the Business Student and Employer Information Form (Appendix-5) is the document that students who do an internship outside of a public institution and receive a salary from the institution where they do their internship will bring at the end of the internship. It is not required among the documents to be submitted for internship start. Who will fill it out is explained in the section on filling out and submitting the internship file.
C) Submission of Internship Start Documents:
Students who have completed their documents must submit their documents to Student Affairs before the internship start date (by 03 JUNE 2022 at the latest) (Internship acceptance form (Appendix-2), Health Insurance Declaration Form (Appendix-3), Otherwise, they cannot start the internship. Students whose documents are complete can send their internship start documents to student affairs by cargo by the final submission date (03 JUNE 2022).
D) Filling and Submission of Internship File:
1. The internship file will be obtained from the student affairs of our Vocational School.
2. The pages where the work done is written in the file will be filled daily and stamped and signed by the institution official at the bottom. (30 pages will be filled due to 30 working days of internship, pages will not be skipped for holidays.
3. The practical workdays page will be filled, and the student signature and institution official stamp and signature will be present.
4. A student photo will be pasted on the file and the confidential evaluation form. (A total of 2 photos will be pasted)
6. The confidential evaluation form inside the file will be filled by the institution, and the stamp and signature must be present. The confidential evaluation form will be cut from its place and put into the envelope on the cover, and the envelope will be sealed and stamped by the institution.
7. The filled files must be physically delivered to student affairs within 7 days at the latest after the end of the internship. Complete files can be sent by cargo.
Note 1: During the internship period, if students obtain a health report they must contact the administration of our Vocational School or the Coordinator lecturer and report the Health Report to the program coordinator and student affairs on the same day.
